Northwest Regional Primary Care Association invites you to submit an abstract for consideration to present at the 2015 Western Forum for Migrant & Community Health.


General Information: The 2015 Annual Western Forum for Migrant & Community Health, to be held in San Diego, CA, February 23-25, provides education and training, information and resource sharing, coalition building, and program and policy development for like-minded community health professionals and their partners.


Submission Deadline: Wednesday, October 1, 2014


Abstract Notification: You will be notified in early November, 2014, via email, whether your abstract is selected. If multiple presenters are listed, only the individual submitting the abstract (the primary presenter) will be notified.


Abstract Selection: Abstracts are peer-reviewed on the basis of quality and clarity of the content, subject relevance (see relevant topics below), and strength of learning objectives.


Relevant Topics: Abstracts should examine current programs, practices, and/or policies related to addressing disparities in health and health care experienced by farmworkers and other vulnerable populations in one of four categories: 1) Health Care Delivery; 2) Policy & Advocacy; 3) Outreach & Health Education; and 4) Workforce & Leadership Development. We are especially interested in: Affordable Care Act and related health care reform initiatives, Community Health Worker workforce, immigration reform, social determinants of health and health equity initiatives, patient-centered medical home, community collaboration, chronic disease management, occupational health, and substance abuse and behavioral health services.
